1. PROTECTED HEALTH INFORMATION (PHI)
WellMed Medical Management, Inc. ("WellMed") collects, uses, and discloses Protected Health Information in accordance with the Health Insurance Portability and Accountability Act ("HIPAA") and applicable state law. This statement describes how PHI about you may be used and disclosed, and how you can access this information.
2. USES AND DISCLOSURES OF PHI
PHI may be used and disclosed for treatment, payment, and health care operations. Treatment includes sharing PHI with other providers involved in your care. Payment includes activities to obtain reimbursement. Operations include quality assessment, training, accreditation, and business management.
3. PROVIDER PORTAL ACCESS
By accessing this portal, you affirm that you are a credentialed provider with a legitimate treatment relationship to the patient whose records you intend to access. Inappropriate access may result in audit review, sanctions, and potential reporting to applicable licensing boards.
4. PATIENT RIGHTS
Patients have the right to inspect and copy their PHI, request amendments, receive an accounting of disclosures, request restrictions on uses and disclosures, request confidential communications, and receive a paper copy of this notice.
5. SECURITY MEASURES
All access to this portal is logged and may be reviewed for compliance purposes. Password sharing is strictly prohibited. Suspected security incidents must be reported to the WellMed Privacy Officer within 24 hours.
6. COMPLAINTS
Complaints regarding privacy practices may be filed with WellMed's Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights. No retaliation will result from filing a complaint.
7. CHANGES TO THIS NOTICE
WellMed reserves the right to change the terms of this notice. New notice terms will be effective for all PHI maintained at the time of the change. Updated notices will be made available through the provider portal.
